This role is primarily remote, allowing for flexible work arrangements while enabling you to engage with clients in the comforting environment of their own homes. Working from a hospice center, you will become an integral part of a multidisciplinary team, collaborating with physicians, nurses, and other health care providers to foster holistic care. Your contributions will directly influence the quality of life for clients navigating challenging health circumstances.

• Conduct thorough assessments and engage with clients experiencing major mental illnesses and severe emotional disorders, employing evidence-based practices.
• Develop integrated care plans tailored to individual client needs, while ensuring seamless access to complementary services and resources.
• Facilitate individual, group, and family therapy sessions aimed at promoting recovery and building resilience among clients.
• Provide preventive, focused treatment services to clients, utilizing a range of therapeutic interventions to address their unique concerns.
• Maintain thorough documentation and progress notes in accordance with organizational standards, ensuring timely completion of reports and data submissions.
• Participate in crisis intervention efforts when necessary, utilizing de-escalation techniques and appropriate resource referrals.
• Work collaboratively with team members to establish common goals, drive quality improvement initiatives, and enhance service delivery.

This role primarily supports clients residing in hospice care who are facing complex medical and psychological challenges. You will work with individuals from diverse backgrounds and cultures, nurturing relationships built on trust, respect, and understanding. • Active and unrestricted LCSW licensure in the state of Tennessee.
• Bachelor’s Degree in Social Work or related field from an accredited institution.
• A minimum of 2 years of clinical experience, particularly in mental health or social services.
• Strong communication skills and the ability to establish rapport with clients and team members.
• Familiarity with various therapeutic modalities and interventions.
• Proven ability to adhere to ethical standards and professional guidelines while delivering compassionate care.

• Experience in hospice or palliative care settings.
• Knowledge of community resources and service linkages for under-served populations.
• Proficiency in remote service delivery platforms and electronic health record (EHR) systems.

The salary range for this position is USD 70,000 – 85,000 per year, commensurate with experience and qualifications.
