• Conduct comprehensive psychosocial assessments to identify patients’ underlying behavioral health needs and develop individualized care plans that address these needs.
• Collaborate with a multidisciplinary medical care team to provide integrated treatment that encompasses both mental and physical health considerations, ensuring a holistic approach to patient care.
• Utilize evidence-based treatment methodologies to deliver effective mental health interventions, including therapeutic sessions that cater to the unique circumstances of each patient.
• Evaluate and leverage community resources, guiding patients and their families to support systems that can enhance their quality of life and coping mechanisms during hospice care.
• Maintain accurate and timely documentation of patient interactions, treatment plans, and outcomes in accordance with organizational policies and applicable regulations.
• Provide staff training and consultation on psycho-social issues, enhancing team awareness and strategies for addressing behavioral health concerns.
• Stay informed about changes in mental health practices and integrate culturally competent strategies to effectively treat diverse patient populations.

The primary focus of this role will be on hospice care patients, who typically include individuals with terminal illnesses and their families. You will encounter a diverse demographic ranging in age from pediatric to geriatric populations, each with unique psychosocial challenges associated with end-of-life issues. Your expertise in mental health will be crucial in facilitating conversations around grief, loss, and emotional resilience for both patients and their loved ones.

• Master's Degree in Social Work (MSW) from an accredited institution.
• Active licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington.
• Minimum of 2 years of professional experience in social work, specifically with a focus on psychiatric care and behavioral health.
• Certification in the safe management of disruptive and assaultive behavior is required.

• Experience working in hospice or palliative care settings is highly desirable.
• Familiarity with electronic health records (EHR) and telehealth platforms to optimize patient engagement.
