Unified Care NW is actively seeking a dedicated and experienced Licensed Independent Clinical Social Worker (LICSW) to provide comprehensive social work services within our remote hospice center located in Marysville, Washington. This full-time role offers an annual salary range of $100,000 - $105,000 and support for flexible remote work arrangements while ensuring optimal patient care and support for families navigating end-of-life issues.

• Provide in-depth clinical assessments and psychotherapeutic interventions to hospice patients and their families, utilizing recognized evidence-based treatment models to address bereavement, chronic illness, and psychosocial challenges.
• Implement and monitor a care plan tailored to each patient’s needs, ensuring comprehensive communication and collaboration with other healthcare providers, caregivers, and community resources.
• Foster supportive relationships with clients, empowering them through education about their rights and health-related resources that promote autonomy.
• Facilitate ongoing evaluations and necessary adjustments to care plans, particularly regarding legal and compliance matters, helping clients to navigate legal responsibility agreements (LRA) and extension processes in coordination with the clinical review team (CRT).

You will primarily support patients receiving hospice care and their families, who may be facing emotional distress, complex family dynamics, or financial hardships. This role requires a nuanced understanding of grief, culture, and socioeconomic diversity to provide appropriate support that respects each client's unique background and experiences.

• A Master's degree in social work (MSW) and current LICSW licensure in the state of Washington.
• Minimum of three years of relevant social work experience, preferably in hospice, palliative care, or mental health settings.
• Demonstrated ability to navigate Medicaid, Medicare, and private insurance systems, along with knowledge of community resources for disadvantaged populations.
• Exceptional interpersonal skills, with an ability to work effectively with individuals from diverse cultural and socioeconomic backgrounds.

• Hands-on experience with clients experiencing homelessness, along with a clear understanding of their unique challenges and available resources.
• Familiarity with integrating behavioral health assessments within primary care settings, though training will be provided to qualified candidates lacking this experience.
• Experience in collaborative environments and the ability to engage with schools, social service agencies, and relevant stakeholders in client care.
