• Conduct comprehensive clinical assessments with patients in hospice care to determine their psychological, social, and emotional needs, utilizing solution-based strategies to develop individualized treatment plans.

Your primary focus will be on patients receiving hospice care services, which includes working with individuals dealing with chronic illnesses, terminal diagnoses, and varying levels of mental health challenges. You will support families through crisis counseling, grief support, and effective communication strategies that respect the wishes and preferences of each individual in palliative care.

• A Master’s degree in Social Work from an accredited program.

• Current and valid LICSW license in Massachusetts.

• A minimum of 2 years of clinical experience, specifically in hospice care or similar settings, emphasizing psychotherapy.
