Part-Time Hospice RN Case Manager

Best Hospice Care of Texas is looking for a compassionate, dependable, and patient-focused Part-Time RN Case Manager to join our growing team!

This is a great opportunity for an RN who loves building relationships with patients and families, values having the time to provide truly personal care, and wants to make a difference without taking on a traditional full-time position.

What You'll Do

As an RN Case Manager, you'll be an important part of each patient's hospice journey. You'll work closely with patients, families, caregivers, and our interdisciplinary team to make sure every patient receives compassionate, coordinated care.

Your responsibilities will include:

  • Providing skilled nursing care to hospice patients in their homes or care settings
  • Completing patient assessments and developing individualized plans of care
  • Managing and coordinating care for your assigned patients
  • Monitoring changes in condition and communicating updates with the hospice team and physician
  • Providing education, guidance, and emotional support to patients and their families
  • Managing symptoms and helping patients remain as comfortable as possible
  • Completing required clinical documentation accurately and timely
  • Participating in interdisciplinary team collaboration and care planning
  • Communicating with CNAs, social workers, chaplains, providers, and other members of the care team
  • Helping ensure patients and families feel supported, informed, and cared for throughout their hospice experience

What We're Looking For

We're looking for someone who is:

  • A current Texas Registered Nurse (RN)
  • Compassionate, dependable, and comfortable working independently
  • A strong communicator and team player
  • Organized and able to manage multiple patient needs
  • Comfortable traveling to patients within the assigned service area
  • Familiar with hospice, home health, or case management preferred, but not required
  • Ready to bring a positive attitude and a heart for patient-centered care
