The Licensed Clinical Social Worker will:

• Establish trusting relationships with clients through active listening and empathetic communication.
• Conduct evidence-based screenings, assessments, and therapeutic interventions tailored to individual client needs.
• Create and implement integrated care plans that promote access to necessary services and foster collaboration among clients, families, and healthcare providers.
• Monitor client progress and adjust treatment plans as necessary, ensuring services are responsive to each client's evolving circumstances.
• Facilitate individual, group, and family therapy sessions, focusing on coping strategies, skill development, and emotional support.
• Engage in crisis intervention efforts, assessing situations promptly to provide immediate and effective support.
• Participate actively in team meetings and case discussions to improve service delivery and outcomes.
• Maintain accurate and timely documentation of client interactions and service delivery metrics according to organizational standards.
• Provide on-call coverage as needed to support clients outside of regular working hours.
