As a Licensed Clinical Social Worker at Healing Springs Family & Wellness Center, your day-to-day responsibilities will include:

• Conducting comprehensive psychosocial assessments to evaluate patients' emotional, social, and financial needs.
• Providing individual and group counseling sessions focused on topics such as adjustment to illness, grief, and coping strategies.
• Collaborating closely with hospice care teams to develop and implement personalized care plans that address holistic patient needs.
• Coordinating post-discharge services with community organizations to ensure continuity of care and access to necessary resources.
• Delivering crisis intervention and support to patients and families facing emotional challenges during their hospice journey.
• Maintaining clear and accurate documentation in compliance with hospital policies and legal requirements.
• Participating actively in team meetings and contributing to the development of hospital policies related to social work practices.

Your primary focus will be supporting patients in hospice care, including those dealing with terminal illnesses and their families. Through empathetic listening and tailored interventions, you will address grief, anxiety, and other psychological burdens, thereby enhancing the quality of life for both patients and their loved ones. You will encounter diverse patient backgrounds, requiring cultural competence and adaptability in your approach.

The ideal candidate will possess the following qualifications:

• A Master’s degree in Social Work from an accredited program.
• Current active licensure as a Licensed Clinical Social Worker (LCSW) in California.
• Strong foundational knowledge of medical terminology, mental health disorders, and crisis intervention techniques.
• Exceptional communication, interpersonal, and collaboration skills, allowing for effective interactions with patients, families, and health care teams.

While not mandatory, candidates will find themselves more competitive with:

• A minimum of two years of experience working in a healthcare or hospice setting.
