As part of the hospice care team, you will engage with clients facing end-of-life challenges and their families, providing both emotional and psychological support. • Conduct secure and confidential remote therapy sessions with individuals, utilizing a robust telehealth platform that facilitates effective communication and engagement.
• Perform thorough assessments to evaluate clients' emotional, social, and mental health needs, enabling the development of tailored treatment plans and therapeutic interventions.
• Provide immediate crisis intervention and support, ensuring the safety and well-being of clients during emotionally challenging situations.
• Guide clients in connecting with vital community resources, support systems, and social services to bolster their overall well-being and address practical care needs.
• Maintain meticulous electronic client records and progress notes, adhering to legal guidelines and ethical standards to ensure confidentiality.
• Collaborate effectively with interdisciplinary teams, promoting a holistic approach to care that encompasses the diverse needs of clients and their families.
• Educate clients on coping mechanisms and self-care strategies, empowering them to better manage their mental health and emotional challenges.

In this role, you will primarily work with clients in a hospice setting, where sensitivity and understanding are paramount. The focus will be on supporting individuals dealing with complex emotions surrounding life-limiting illnesses and offering guidance to families coping with grief and loss. Your expertise will help clients navigate these challenging times, fostering a space where they feel heard and understood.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.
• A Master's degree in Social Work (MSW) or a related field is required to ensure a strong foundation in therapeutic practices.
• A minimum of 3 years of relevant experience is required, with special consideration given to applicants with hospice or end-of-life care experience.
• Proficiency with telehealth technologies and platforms to deliver effective remote services.
• Strong interpersonal skills that foster effective communication and rapport-building in remote interactions.
• Commitment to ethical practice and confidentiality in all aspects of client care.
• Ability to work independently and manage a full caseload in a remote work environment.
• Flexibility to accommodate diverse client availability across different time zones.

• Previous experience in hospice care or working with terminally ill patients is highly desirable.
• Familiarity with grief counseling and bereavement support techniques.
• Experience with electronic health record (EHR) systems and documentation processes.
