Schedule: Flexible | Choose your own hours | Day/Evening shifts | Monday-Friday | Weekends as needed.
Position Overview
PharmD Live is seeking an experienced Clinical Pharmacist to join our expanding telehealth team in Virginia. In this role, you'll deliver pharmacist-led care coordination and medication management services that reduce risk, improve outcomes, and advance value-based care performance. You'll support patients remotely through Chronic Care Management (CCM), Transitions of Care Management (TCM), Remote Patient Monitoring (RPM), and Medication Therapy Management (MTM), ensuring clinical quality and patient satisfaction.
Primary Duties

  • Provide disease and medication management through phone and virtual consultations.
  • Complete comprehensive medication reviews and support chronic disease management.
  • Identify and resolve medication-related problems to enhance safety and effectiveness.
  • Recommend evidence-based therapeutic alternatives and collaborate with medical teams.
  • Document all patient encounters accurately within our clinical care platform.
  • Support transitions of care through medication reconciliation and follow-up.
  • Educate patients and caregivers on medication adherence and health literacy.
  • Maintain awareness of evolving best practices and regulatory requirements.
  • Offer clinical insights and leadership to internal stakeholders.
  • Participate in ongoing performance improvement and outcome reporting.
Required Qualifications
  • PharmD degree from an ACPE-accredited institution.
  • Licensed or eligible for licensure as a pharmacist in Virginia.
  • Board Certification (BCGP, BCPS, BCACP) preferred or obtainable within 12 months.
  • Strong clinical reasoning and patient-centered communication skills.
  • Working knowledge of chronic disease management and value-based care.
  • Proficient in Google Workspace and clinical software documentation.
  • Self-motivated, organized, and adaptable to a virtual care environment.
