What is a remote geriatric pharmacist?

A remote geriatric pharmacist is a licensed pharmacist who specializes in the medication management of older adults, providing their services virtually rather than in person. They review prescriptions, monitor for drug interactions, consult with patients and healthcare providers, and help optimize medication regimens to improve patient outcomes. Working remotely, they use telehealth platforms, electronic health records, and other digital tools to deliver care. This role is especially valuable for elderly patients with complex medication needs or limited mobility.

What are the typical responsibilities and collaboration methods for a remote geriatric pharmacist?

As a Remote Geriatric Pharmacist, your primary responsibilities include reviewing medication regimens for elderly patients, providing medication therapy management, and ensuring safe, effective drug use tailored to geriatric needs. You will regularly communicate with physicians, nurses, and caregivers via secure digital platforms to discuss patient cases and offer recommendations.
