On Call Geriatric Pharmacists are licensed pharmacists who specialize in the medication needs of older adults and are available to provide expertise outside of regular business hours. They help manage complex medication regimens, prevent drug interactions, and ensure safe and effective use of medicines for geriatric patients. These pharmacists often work closely with healthcare teams in settings such as hospitals, nursing homes, or through telehealth, providing consultations and support as needed, including during nights, weekends, or emergencies.

What are the key skills and qualifications needed to thrive as an on call geriatric pharmacist?

To thrive as an On Call Geriatric Pharmacist, you need a Doctor of Pharmacy (PharmD) degree, state licensure, and specialized knowledge in geriatric pharmacotherapy. Familiarity with pharmacy management systems, drug interaction databases, and electronic health records is typically required. Strong attention to detail, excellent communication, and the ability to make quick, informed decisions in urgent situations are essential soft skills. These competencies are crucial for ensuring safe, effective medication management and timely support for elderly patients, often in high-pressure and after-hours scenarios.

What are the typical challenges faced by an on call geriatric pharmacist, and how can they be managed?
