We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join the Foster Future Foundation team. This position is remote, providing flexibility while allowing you to focus on client interactions and case management. You will be integrated into a dynamic hospice center, collaborating with healthcare professionals and other social workers to ensure that clients receive holistic care. Although the role allows for remote work, you will also participate in occasional team meetings and training sessions as well as community outreach efforts to raise awareness of agency services.

• Conduct comprehensive assessments to identify and evaluate clients’ social, emotional, and environmental challenges that impact their health and well-being.
• Develop tailored service plans that include specific goals, intervention strategies, and methods for monitoring progress.
• Provide compassionate counseling and support to individuals and families as they navigate life’s challenges, ensuring they feel heard and respected.
• Collaborate with clients to create personalized service plans, facilitating connections to vital community resources and support systems.
• Administer crisis intervention, perform risk assessments, and engage in safety planning when necessary to ensure the immediate well-being of clients.
• Work collaboratively with community agencies and organizations to coordinate services, ensuring comprehensive advocacy on behalf of clients.
• Engage in community outreach initiatives to promote the foundation’s mission and services, raising awareness of social issues affecting our clientele.
• Participate actively in case conferences, team meetings, and ongoing professional development opportunities to enhance skills and knowledge.

Your primary focus will be on individuals and families experiencing significant life challenges, particularly those in end-of-life situations or facing serious health concerns. This includes providing therapeutic support for emotional distress, facilitating communication with family members, and helping clients navigate complex healthcare systems. Understanding the unique needs of this patient population is essential in delivering effective and culturally competent care.

• A Master’s degree in Social Work from an accredited program is required.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
• Demonstrated knowledge of social and economic justice issues, along with community resources relevant to client support.
• Strong understanding of trauma-informed care principles, applying them in practice to promote healing and resilience.
• Exceptional communication skills and cultural competence, with a commitment to addressing diversity and inclusion in service delivery.

• Experience working in a social service agency, particularly in hospice or palliative care settings.
• Knowledge of evidence-based practices and methodologies in social work and counseling.
• Familiarity with community outreach strategies and collaborative partnerships.
