Who are you?

As our Patient Retention CEP, you will collaborate with the clinical team to enhance patient retention and conversion strategies. This role will proactively reach out to patients who are at risk of exiting the program to address their concerns. As a Patient Retention CEP, you will leverage your passion for cardiopulmonary rehabilitation, patient relationship skills, and sales expertise to demonstrate the long-term benefits of our program to patients. You will play a key role in Carda Health's growth, as you truly believe in the power of telehealth to transform the way Americans receive healthcare.

Responsibilities:

  • Work collaboratively with the clinical team and founders to improve our cutting edge digital care delivery model and develop patient retention strategies.
  • Take pride in delivering high quality care for patients going through our cardiac rehab program and our clinical studies.
  • Be knowledgeable of our different clinical programs, protocols, and policies.
  • Improved patient engagement and conversion rates through targeted outreach to highlight program progress, addressing concerns, and reinforcing program benefits.
  • Utilize motivational interviewing to address various financial/exercise barriers to continued participation in our program.
  • Provide support to our patients, empowering them to be accountable and take ownership of their rehabilitation.
  • Work collaboratively with our technology team to support a smooth experience for our patients and clients.
  • Be willing to do whatever it takes to ensure a great, engaging rehab experience for our patients.

Qualifications:

  • Minimum 1+ years of relevant clinical experience and top performance working as an Exercise Physiologist in Cardiopulmonary Rehab.
  • Strong proficiency working with mobile technology - you will be helping guide patients through our digital health programs including troubleshooting their technology issues with wearables and tablets.
  • Proficiency in excel for data tracking
  • Great interpersonal skills that foster a caring environment for patients.
  • Diligence and organization with the ability to create and adhere to repeatable processes.
  • Ability to perform with little direction in a high-growth, early stage startup (ie self-driven).
  • Bachelor's Degree in Exercise Physiology, Master's Degree Preferred.
  • ACSM-CEP certification preferred.
