• Conduct thorough assessments following established protocols to identify the specific needs of each patient, ensuring a person-centered approach to treatment.
• Maintain open lines of communication with supervisors, fellow professionals, family members, and other support systems to discuss patient progress and challenges.
• Create and provide a tentative scheduling framework for appointments, home visits, and observations to monitor patient progress effectively.
• Coordinate and execute home visits as necessary, adhering to guidelines for in-home observations and comprehensive data collection.
• Supervise and ensure the effective implementation of treatment plans tailored to each patient's needs, facilitating continuous improvement in care delivery.
• Engage actively in Interdisciplinary Team Meetings to drive discussions on treatment updates and strengthen collaborative practices.
• Document and complete progress notes on a monthly and quarterly basis to track and evaluate patient outcomes.
• Organize and facilitate training sessions for staff and team members to enhance their competencies relating to implementation of treatment protocols.
• Provide ongoing support through follow-up interviews and home visits to sustain and encourage patients’ progress.
• Maintain accurate and accessible records that ensure information about patients is readily available to authorized parties.
• Act as a liaison between the clinic and the broader mental health community to establish partnerships that support our development goals.

The Licensed Professional Counselor will support a diverse clientele, addressing varied mental health needs including those of adults, youth, families, and couples. A specialization or knowledge in areas such as family therapy, couples therapy, substance abuse, and trauma is greatly beneficial for effectively managing the unique challenges faced by these populations.

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Tennessee.
• A minimum of two years of clinical counseling experience with proven success in providing quality care.
• Strong understanding of legal and ethical standards relevant to mental health practice.
• Excellent interpersonal and communication skills, with the ability to connect with a wide range of individuals.
• Demonstrated ability to work collaboratively with an interdisciplinary healthcare team.
• Familiarity with Electronic Medical Records (EMR) software and related technologies for scheduling, documentation, and patient management.

• Specialization in areas such as family therapy, couples therapy, addiction counseling, or trauma-informed care is highly preferred.
• Previous telehealth or remote counseling experience is advantageous, highlighting adaptability to digital platforms.
