This role is set within a hospice center, allowing for a unique blend of social work and mental health support. While the position is remote, our team functions collaboratively to ensure high-quality care is delivered consistently. We employ state-of-the-art telehealth technologies to facilitate consultations, treatment sessions, and team meetings, reducing barriers and enhancing accessibility for our clients.

• Conduct thorough psycho-social assessments to understand client needs and backgrounds.
• Develop and implement customized treatment plans incorporating evidence-based practices that address specific challenges faced by clients.
• Utilize advanced psychotherapy techniques and interventions aimed at enhancing client resilience and promoting mental wellness.
• Maintain meticulous and confidential clinical documentation in accordance with HIPAA regulations and organizational policies.
• Work collaboratively within a multidisciplinary treatment team to ensure holistic care and coordinate services for patients.
• Provide psycho-education and support to patients and their families, helping them to understand mental health diagnoses, treatment options, and coping strategies.
• Engage in ongoing professional development and stay informed about current research and best practices in mental health support.

Our clients encompass a diverse demographic, including individuals grappling with trauma, anxiety, depression, and situational crises. The social workers will provide culturally competent care, recognizing and respecting the unique backgrounds and experiences of each client. This role involves working closely with various age groups and addressing a broad spectrum of mental health concerns.

• A Master's degree in Social Work from an accredited institution.
• Possession of a valid LCSW license in the state of Illinois.
• At least 2 years of clinical experience in a mental health setting, with a proven track record of successful client outcomes.
• Proficiency in a range of psychotherapeutic approaches, including solution-focused and trauma-informed strategies.
• Exceptional communication and interpersonal skills to facilitate effective interactions with clients and team members.
• Strong analytical and problem-solving skills, capable of adapting approaches based on client needs.
• A collaborative team player who thrives in a remote work environment.

• Experience in hospice or palliative care settings is a plus.
• Familiarity with electronic health record (EHR) systems and teletherapy platforms.
• Background in community outreach or engagement initiatives.
• Understanding of the challenges and opportunities in remote service delivery.

The position offers a competitive salary range of USD 80,000 to 100,000 per year, commensurate with experience and qualifications. In addition to a competitive salary, we provide a comprehensive benefits package that includes:
