Key Responsibilities

As a Licensed Clinical Social Worker at Intimate Bridge Counseling, your responsibilities will include:

• Conducting comprehensive evaluations and assessments for young patients in a hospice setting to determine their emotional, social, and behavioral needs.
• Engaging in the development and implementation of Individualized Family Service Plans (IFSPs) to ensure holistic care and support for children and their families.
• Monitoring patient outcomes and providing feedback to the multidisciplinary team to adjust treatment strategies that promote positive results.
• Establishing strong rapport with children and families, promoting a nurturing and secure environment that encourages open communication.
• Utilizing evidence-based practices to deliver effective therapeutic interventions tailored to each child’s unique circumstances.
• Providing ongoing support and educational resources to families, equipping them with strategies to enhance their child’s development.
• Complying with all applicable Federal and State regulations, maintaining the highest standards for health and safety in service provision.

Patient Population / Client Focus

Qualifications

• A Master's Degree or Doctorate in Social Work (MSW or DSW) is required.
• Current licensure as a Licensed Clinical Social Worker (LCSW) issued by the Department of Public Health.
• A valid driver's license is necessary to meet any potential assessment or community engagement needs.
• Proficient in addressing various learning and communication styles to adequately support children and their families.
