The role of Licensed Clinical Social Worker is remote but anchored in a collaborative and supportive environment. You will engage with an interdisciplinary team, including nurses, caregivers, and healthcare professionals, ensuring a holistic approach to client care. Regular video conferences and virtual meetings facilitate continuous teamwork, while you have the flexibility to manage your schedule around client needs. Full-time hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend shifts as required.

• Conduct comprehensive assessments of clients' psychosocial needs, integrating emotional, mental, and social factors into your evaluations.
• Collaborate with clients and their families to design personalized care plans tailored to their unique goals and circumstances.
• Provide empathetic counseling and emotional support during challenging life transitions, ensuring clients feel understood and validated.
• Advocate for clients to receive their rightful access to community resources and services, empowering them to make informed decisions.
• Work interdependently with a diverse team of healthcare providers to enhance care options and address clients' multifaceted needs.
• Attend to clients' well-being through regular virtual visits, assessing their mental and emotional health while addressing any arising safety concerns.
• Facilitate transparent communication among family members involved in caregiving, helping to resolve any potential conflicts or misunderstandings.
• Educate clients and families about community resources, support groups, and programs to improve their social engagement and overall quality of life.
• Maintain meticulous and timely documentation of client assessments, care plans, and progress to ensure proper continuity of care.
• Keep abreast of current social work practices, ethical guidelines, and regulatory requirements to uphold the highest standards of service.

• A valid and current state licensure as a Licensed Clinical Social Worker (LCSW).
• A Master’s degree in Social Work from an accredited institution.
• A minimum of two years of progressively responsible social work experience, demonstrating expertise in community or geriatric settings.
• In-depth knowledge of aging-related issues, social work principles, and community resources available for seniors.
• Exceptional skills in assessment, counseling, and problem-solving that translate into effective client care.
• Strong communication and interpersonal abilities to foster collaboration within an interdisciplinary team.
• Proficiency in using electronic health record (EHR) systems to manage client information and communication.

• Experience working in a remote capacity with an emphasis on telehealth services.

Bliss Reformation And Wellness Center offers a competitive salary ranging from $90,000 to $100,000 annually.
