Job Title: Senior LTSS Care Manager (RN)
Location: Remote, Texas (Candidate should be from Austin area)
Duration: 6-Month Contract with Potential Extension
About the Role
We are seeking an experienced Registered Nurse (RN) to join a care management team supporting members with complex and high-acuity healthcare needs. In this role, you will coordinate medical and supportive services, develop personalized care plans, conduct in-home member visits, and help improve health outcomes through quality, cost-effective care.
Key Responsibilities
  • Assess member needs and coordinate care across the continuum of services
  • Develop and manage individualized care/service plans
  • Conduct in-home member visits and ongoing care evaluations
  • Educate members and caregivers on available benefits, services, and care options
  • Maintain timely and accurate documentation in accordance with established standards
  • Collaborate with healthcare providers, caregivers, and community resources
Requirements
  • Active Registered Nurse (RN) license
  • Associate Degree in Nursing (ADN), Bachelor of Science in Nursing (BSN), or Master of Science in Nursing (MSN)
  • Experience in care management, case management, or long-term services and supports (LTSS)
  • Ability to conduct in-home member visits and travel locally within assigned territory
  • Strong documentation, communication, and organizational skills
  • Ability to meet performance and compliance metrics
Preferred Qualifications
  • Experience working with pediatric populations
  • Managed care or health plan experience
  • Background supporting complex or high-acuity member populations
  • Knowledge of community-based and long-term care services
Work Schedule
  • Monday-Friday, 8:00 AM-5:00 PM
  • Approximately six member visits per week
