• Conduct thorough clinical assessments to understand each patient’s unique situation and develop comprehensive, personalized treatment plans.
• Utilize evidence-based therapeutic interventions to address issues such as grief, anxiety, and depression related to life-limiting illnesses.
• Maintain accurate clinical records in a timely manner, ensuring compliance with established guidelines and best practices.
• Collaborate with other healthcare professionals, including nurses, physicians, and chaplains, to develop and implement a holistic care plan.
• Provide education and emotional support to both patients and families, aiding them in navigating the complexities of hospice care.

Your work within the Hospice Center will focus on providing support to patients who have been diagnosed with terminal illnesses, along with their families and caregivers. This role requires sensitivity and skill in addressing emotional distress, grief, and family dynamics during challenging times. Your social work expertise will be pivotal in helping these individuals process their emotions and prepare for the end-of-life journey.

• Master’s degree in Social Work from an accredited institution is mandatory.
• A valid LCSW license in the state of Maryland is essential to practice in the clinical setting.
• Strong knowledge of solution-focused psychotherapy methods and hospice care dynamics.
• Exceptional communication skills, with the ability to establish rapport and trust with patients and families.
• Proficient in maintaining confidentiality and complying with healthcare regulations.

• A minimum of 2 years of clinical experience, specifically in hospice or palliative care settings, is highly desirable.
• Experience in crisis intervention and family counseling will enhance your candidacy.
• Familiarity with electronic health records and telehealth systems is a plus, ensuring efficient and organized patient management.
