  • Advocate for clientsโ€™ rights and assist with navigating healthcare systems and community resources to ensure comprehensive support.
  • Maintain thorough and confidential client records, ensuring compliance with legal and ethical standards while documenting assessments, treatment plans, progress notes, and outcomes.
Patient Population / Client Focus

In this role, you will primarily support individuals and families within our community health clinic framework, serving a diverse population with varying backgrounds and experiences. Qualifications
