Although you will primarily work from home, you will collaborate closely with a diverse team of healthcare professionals through virtual meetings and telehealth platforms to ensure a cohesive approach to patient care.

• Conduct in-depth psychosocial assessments to evaluate the emotional, social, and environmental factors impacting patients and their families, tailoring interventions accordingly.
• Develop, implement, and continuously evaluate individualized treatment plans utilizing evidence-based social work practices and therapeutic modalities.
• Collaborate with healthcare providers, including physicians and case managers, to formulate effective discharge plans that address patients' ongoing needs post-hospitalization.
• Facilitate crisis intervention and provide therapeutic support through brief and long-term individual, group, and family therapy sessions.
• Educate patients and their families on coping strategies and resources available to assist in adjusting to health-related challenges.
• Support interdisciplinary efforts by participating in case discussions and sharing expertise on psychosocial factors affecting patient care.
• Participate in an on-call rotation, ensuring availability to address urgent after-hour patient needs.
• Engage in community outreach through education, consultation, and research to promote resources for cancer survivors and enhance community support systems.
• Contribute to the professional development of the field by mentoring new social workers and participating in educational conferences.

As a Licensed Clinical Social Worker at Healing Garden, you will primarily work with adult patients facing significant emotional and psychological challenges associated with chronic illnesses, particularly oncology. Your expertise will help address issues such as depression, anxiety, and adjustment disorders that may arise in this vulnerable patient population. Additionally, you will provide educational resources and counseling to family members, enhancing their understanding and ability to support their loved ones.

• A Master's degree in Social Work (MSW) with a current Licensed Clinical Social Worker (LCSW) license in the state of Colorado.
• Demonstrated experience in conducting psychosocial assessments and providing therapeutic interventions in a healthcare setting.
• Strong understanding of the psychosocial impacts of chronic illnesses and the associated treatment processes.
• Excellent communication skills, both written and verbal, with the ability to build rapport with patients from diverse backgrounds.

• Previous experience working with oncology patients and familiarity with the unique challenges faced in this area of care.
• Experience in crisis intervention, behavioral therapy, and facilitating group therapy sessions.
• Knowledge of community resources available to patients and families affected by chronic illness.

Healing Garden Behavioral Health Center offers a competitive salary ranging from USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
