Position Overview

Gotham Enterprises Ltd is seeking a dedicated and skilled Licensed Clinical Social Worker (LCSW) to join our remote team, providing essential support to seniors through our Community Health Clinic. Work Environment

This position is entirely remote, allowing for flexible scheduling and the ability to provide services from the comfort of your own home. The work hours are primarily Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours required based on client needs. You will have the opportunity to conduct assessments, provide counseling, and collaborate with other health professionals while maintaining an open line of communication using video conferencing and electronic health records (EHR) systems.

Key Responsibilities

The key responsibilities of the Licensed Clinical Social Worker (LCSW) include:

• Conducting comprehensive assessments of clients’ psychosocial needs, considering their emotional, mental, and social challenges.
• Developing personalized care plans in partnership with clients and their families that are tailored to their specific goals and needs.
• Providing counseling and emotional support to assist clients and families in navigating life transitions and challenges, including health concerns and end-of-life decisions.
• Advocating for clients’ rights and coordinating access to community resources, ensuring they receive the necessary support services.
• Collaborating with an interdisciplinary team of healthcare professionals such as nurses and caregivers to ensure an integrated approach to client care.
• Monitoring clients' well-being through regular virtual visits, assessing their living conditions, and addressing any emerging concerns.
• Facilitating communication among family members to resolve conflicts and enhance the care experience for the client.
• Maintaining detailed and accurate documentation of assessments, care plans, and client interactions to support continuity of care.
• Staying informed about the latest social work practices, regulations, and ethical standards to uphold quality service delivery.

Patient Population / Client Focus

This role primarily focuses on the senior population within our community. The clients you will serve often face multiple challenges, including chronic health conditions, social isolation, and significant life transitions. Your expertise in understanding their unique needs will be critical in developing strategies to improve their quality of life and ensuring they feel valued and supported.

Qualifications

To qualify for this role, candidates must possess:

• A Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of direct experience in social work, preferably within a community health setting or geriatric care.
• A thorough knowledge of aging-related issues, social work principles, and available community resources.
• Strong assessment and counseling skills, with a demonstrated ability to empathize and establish rapport with clients and their families.
• Effective communication and interpersonal skills to foster collaboration with diverse teams.
• Proficiency in electronic health record (EHR) systems and familiarity with digital communication tools.

Preferred Experience

Ideal candidates will have experience working specifically with senior clients, an understanding of geriatric care, and additional training in counseling techniques. Familiarity with community resources available for seniors and a background in advocacy will also be beneficial.

Compensation and Benefits
