This position operates remotely, providing you the flexibility to make a significant impact from a distance. However, you will collaborate closely with a multidisciplinary team at our Detox Center, fostering a supportive environment that welcomes innovative ideas to enhance service delivery. Your role will engage with individuals suffering from addiction and co-occurring mental health issues, ensuring that appropriate interventions are effectively implemented.

• Provide direct clinical services to clients both in virtual settings and through community outreach, addressing complex mental health and emotional issues.
• Conduct thorough client assessments, utilizing evidence-based screening tools and methods to identify needs effectively.
• Develop and implement personalized care plans in collaboration with clients, families, and interdisciplinary team members, ensuring all aspects of client care are coordinated.
• Facilitate individual, group, and family therapy sessions tailored to the unique needs of clients.
• Monitor case management processes, including client admissions, treatments, and discharges, ensuring consistent communication among healthcare providers.
• Participate actively in safety assessments, identifying potential risks and implementing strategies to mitigate them proactively.
• Contribute to quality improvement initiatives by participating in organizational committees focusing on enhancing service delivery and patient outcomes.
• Maintain accurate and timely documentation of client interactions and progress, ensuring compliance with ethical guidelines and organizational policies.

Your primary focus will be on individuals struggling with severe emotional disorders and substance use challenges, many of whom come from marginalized backgrounds. This role requires cultural competency and the ability to connect with clients from diverse traditions and life experiences, fostering trust and open communication to encourage engagement in their recovery journey.

• Master's degree in Social Work from an accredited institution, with an active LCSW license in the state of Connecticut.
• Expertise in assessment, diagnosis, and treatment methodologies specific to mental health and substance use populations.
• Exceptional verbal and written communication skills, crucial for effective clinical documentation and team collaboration.
• Strong organizational skills and the ability to manage a caseload efficiently.
• Commitment to maintaining high ethical standards and adhering to COHC's code of conduct.

• A minimum of 2 years of clinical experience, preferably within a detox or substance use recovery setting.
• Experience working with diverse populations and familiarity with age-specific developmental considerations in treatment.
• Proven ability to engage clients in crisis situations and manage risk effectively.

The salary range for this role is competitive, between USD 70,000 and 85,000 per year, commensurate with qualifications and experience.
