As a part of our community health clinic, you will work within an interdisciplinary team of healthcare professionals including medical doctors, nurses, and allied health specialists, focused on delivering comprehensive and coordinated care. • Conduct thorough psychosocial assessments to evaluate patients' emotional, social, and financial circumstances, informing treatment considerations.
• Engage in individual and group therapy sessions focusing on issues such as adjustment, grief, and coping strategies to guide patients through their experiences.
• Coordinate closely with medical teams to develop and implement integrated treatment plans, directly impacting patient care and outcomes.
• Facilitate discharge planning by developing comprehensive plans that connect patients to necessary community resources to ensure ongoing support post-discharge.
• Participate in collaborative case meetings, advocating for patient needs and contributing your social work expertise to optimize care delivery.
• Provide crisis intervention and emotional support, empowering patients and families during challenging times.
• Maintain precise and timely documentation in accordance with all legal and organizational standards, supporting quality assurance and accountability.

You will primarily serve a diverse patient population, including individuals from various socio-economic backgrounds, ages, and health conditions. By providing culturally competent care, you will address the unique needs of each patient, ensuring equitable access to resources and emotional support. Your ability to adapt your communication and methods to meet the distinct needs of each client will be key in achieving positive health outcomes.

• Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong proficiency in conducting psychosocial assessments and providing therapeutic interventions.
• Familiarity with community resources and the ability to connect clients with appropriate services.

• At least two years of clinical experience within a healthcare or hospital environment.
• Demonstrated understanding of medical terminology, mental health disorders, and crisis intervention techniques.
• Proven ability to collaborate effectively with interdisciplinary teams for patient-centered care.
