• Conduct thorough psychosocial assessments to identify various patient needs, focusing on emotional, social, and financial challenges that may impact their healthcare journey.
• Provide individualized and group counseling sessions to address issues such as adjustment disorders, grief, and coping mechanisms related to health challenges.
• Collaborate with interdisciplinary medical teams, ensuring that social determinants of health are considered in patient care plans.
• Coordinate with local community organizations to facilitate post-discharge services, guaranteeing seamless transitions and ongoing support for patients.
• Participate in crisis intervention scenarios, offering immediate support to patients and families facing acute stress due to illness.
• Develop and implement personalized discharge plans that connect patients with necessary resources, enhancing their post-care experience and improving long-term outcomes.
• Engage in continuous professional development by attending hospital committees and presenting social work insights that contribute to the overall enhancement of service delivery.
• Maintain accurate and timely documentation in compliance with all relevant policies and legal guidelines, ensuring that the integrity of patient records is upheld.

The LCSW will address the diverse needs of our patient population, which includes individuals dealing with chronic illnesses, mental health disorders, and situational crises. • Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) without restrictions.

• Exceptional communication and interpersonal skills, allowing for effective collaboration with medical professionals and compassion in patient interactions.

• A minimum of two years of experience working in a healthcare or hospital environment is preferred.
• Experience with electronic health records (EHR) systems to facilitate documentation and care coordination.
• Familiarity with community resources and tools available for patient support, enhancing the quality of care provided.
