• Conduct thorough client assessments to identify emotional, psychological, and social needs, establishing a foundation for personalized treatment plans.
• Implement evidence-supported therapeutic services including individual, family, and group therapy sessions, adaptable to a variety of clinical situations.
• Maintain clear, comprehensive clinical records in accordance with healthcare regulations, ensuring privacy and compliance while enhancing quality of care.
• Collaborate with a multidisciplinary team, including nurses, doctors, and other social workers, to deliver well-rounded and holistic care that addresses both mental and physical health needs.
• Provide educational resources and support for clients and their families, equipping them with coping strategies and practical tools for navigating health challenges.
• Utilize innovative therapeutic approaches such as mindfulness, art therapy, or cognitive behavioral methods to engage clients meaningfully.

This role primarily focuses on individuals in hospice care, often facing life-limiting conditions. You will work with a diverse clientele, including those from various cultural backgrounds and age groups. Your expertise will guide clients and families through complex emotional landscapes while promoting hope and healing.

• A Master’s degree in Social Work from an accredited institution, with an active Licensed Clinical Social Worker (LCSW) certification in California.
• A minimum of two years of relevant clinical experience is preferred, which will enhance your ability to effectively assist clients in varied emotional states.
• Familiarity with mental health issues pertinent to the hospice environment, bolstered by a passion for patient welfare and advocacy.
• Proficiency in using clinical assessment tools and an ability to adapt therapeutic methods to individual client needs.
