As a Remote Clinical Social Worker, your specific responsibilities will include:

• Conducting comprehensive clinical assessments to identify patients’ mental health needs and developing tailored treatment plans guided by evidence-based practices.
• Providing psychotherapy services using modalities such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) that are aligned with the unique needs of patients in hospice care.
• Documenting clinical progress through timely and accurate record-keeping in compliance with HIPAA regulations and organizational standards.
• Engaging in interdisciplinary collaboration with psychiatrists, psychologists, and nursing staff to coordinate holistic patient care and facilitate comprehensive treatment solutions.
• Attending regular peer consultations and case reviews to uphold a high standard of care and initiatives to enhance service delivery.
• Educating patients and their families on mental health concerns and the myriad of treatment options available to create a supportive environment for coping and well-being.
• Remaining current with mental health research and best practices to integrate innovative and effective procedures into clinical practice.

Your role will involve working closely with patients inhabiting various mental health challenges, particularly those in hospice care. This client focus extends to providing emotional support to families navigating the complexities of terminal illness and encouraging them to engage in constructive coping mechanisms.

The ideal candidate will possess:

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license to practice in the state of Illinois.
• A minimum of 2 years of clinical experience, especially in providing psychotherapy to diverse populations.
• Competence in solution-focused and evidence-based therapeutic techniques.
• Exceptional communication and interpersonal skills tailored for developing strong therapeutic relationships.
• A proven ability to work collaboratively within a multidisciplinary team environment.

Experience within hospice care or with patients dealing with end-of-life issues is highly desirable, as this background will empower you to connect meaningfully with clients and guide them through their emotional journeys.
