• Conduct thorough evaluations by reviewing patient health histories to prepare for initial assessments.
• Design and implement individualized mental health care plans based on established clinical practices and patient needs.
• Engage in patient and family interviews to determine educational requirements, treatment objectives, and care processes.
• Assess and document patient and family feedback to enhance treatment plans as necessary.
• Collaborate with physicians and interdisciplinary team members to coordinate comprehensive care strategies and relay ongoing patient status.
• Facilitate appropriate referrals to additional healthcare professionals when required.
• Maintain strict patient confidentiality in compliance with HIPAA regulations.
• Complete necessary documentation within the company’s electronic medical record system, ensuring all cardiac rehabilitation forms are meticulously managed.
• Lead and conduct regular Healthy Minds workshops to provide patients with educational resources and foster engagement.

You will primarily work with diverse patient demographics, including those dealing with mental health issues related to cardiac conditions. Your expertise will help address their psychological and emotional needs, enabling holistic recovery. By providing targeted counseling and education, you will play a crucial role in enhancing patients' quality of life.

• A current and valid LMHC, LCSW, LMFT, or Psychologist license in the state of Florida.
• A Bachelor’s Degree in Psychology, Social Work, or a related field; advanced degrees may be preferred.
• Strong verbal and written communication skills, with a focus on conveying complex information in a clear and concise manner.
• Proficiency with telehealth platforms and electronic medical records systems.
• Commitment to maintaining confidentiality and adhering to ethical standards in practice.

• Prior experience working in a telehealth setting or delivering mental health services remotely.
• Experience in clinical settings that focus on cardiac rehabilitation or chronic illness management.
• Familiarity with group therapy techniques and delivering educational workshops.

We offer a competitive salary ranging from USD 90,000 to $104,000 annually, contingent upon the number of clients seen each week. In addition to an attractive salary, you will receive:
