Key Responsibilities
  • Conduct comprehensive patient evaluations and assessments to determine individual mental health needs and create appropriate treatment plans.
  • Facilitate clear communication among supervisors, direct care personnel, families, and other stakeholders to support patient development and progress.
  • Document treatment progress through regular updates and reports, including monthly and quarterly progress notes.
  • Organize and conduct follow-up interviews, virtual home visits, and in-depth assessments to monitor ongoing patient needs effectively.
  • Help coordinate and lead training sessions for team members to ensure high standards in the execution of the program plan.
  • Participate in interdisciplinary team meetings, collaborating with other health professionals to provide well-rounded support for clients.
  • Maintain precise and secure client records utilizing EMR software to ensure confidentiality and accessibility of information.
  • Actively engage with the mental health community and serve as a bridge to connect patients with available resources and support.
Patient Population / Client Focus

You will work with a diverse client base, including adults and youth, addressing various concerns such as anxiety, depression, trauma, family dynamics, and substance misuse. Qualifications
  • A Master’s or Doctoral degree in Counseling, Psychology, or a related discipline is required.
  • An active and unrestricted Licensed Professional Counselor (LPC) certification within the state of Oregon is mandatory.
  • A minimum of two years of counseling experience, preferably in a clinical setting.
  • Strong understanding of legal and ethical standards governing mental health practices.
