This position is fully remote, allowing for flexibility in your work environment while supporting patients and their families through telehealth modalities. The hospice center environment requires a sensitive and supportive approach, ensuring that even from a distance, you can foster meaningful connections with clients. Technology will play a crucial role in your daily activities, utilizing electronic health records for documentation and teletherapy platforms for client interactions.

• Conduct thorough biopsychosocial assessments, utilizing evidence-based tools to evaluate mental health, substance abuse, and medical needs in alignment with primary care services.
• Provide comprehensive behavioral health services, including individual and group counseling, crisis intervention, intake assessments, and the development of tailored treatment plans.
• Document all client interactions, including progress notes and suicide risk assessments, in a timely and accurate manner within our electronic documentation system.
• Act as a liaison by connecting clients and families with local community resources and support services, including medication assistance, support groups, and educational programs.
• Collaborate with interdisciplinary teams to identify patient needs and contribute to quality improvement initiatives aimed at enhancing care delivery.

The focus of your work will be primarily on individuals who are facing terminal illnesses as well as their families. Your role will involve navigating complex emotional landscapes, addressing grief and loss, and ensuring that our clients receive compassionate, culturally competent care tailored to their specific needs. You will actively support families during challenging times, facilitating discussions around care preferences, and working collaboratively to provide the most appropriate resource systems.

• A valid and active Licensed Clinical Social Worker (LCSW) license in the state of California.
• A Master's degree in Social Work from an accredited institution.
• A minimum of three (3) years of post-licensure clinical experience, including familiarity with hospice and palliative care settings.
• Demonstrated knowledge of counseling techniques, crisis intervention strategies, and the capacity to make independent decisions in a clinical context.
• Strong communication skills and the ability to build rapport with a diverse range of clients.

• Experience in group facilitation and conducting educational workshops in a mental health context.
• Exposure to electronic health record systems and telehealth practices.
• Proven ability to develop and maintain partnerships with community organizations and resources.

The annual salary for this position ranges from $120,000 to $130,000, commensurate with experience and qualifications. In addition to competitive compensation, we offer a comprehensive benefits package that includes:
