As a Licensed Clinical Social Worker at Valley Point Healthcare, your day-to-day responsibilities will include:

• Conducting comprehensive psychosocial assessments to identify patients' emotional, social, and financial challenges, tailoring interventions to address these needs.
• Providing individual and group counseling focused on issues such as adjustment, grief, and coping mechanisms to empower patients during challenging times.
• Collaborating with school personnel and healthcare teams to ensure well-rounded support and consistent communication regarding patients’ treatment plans.
• Coordinating with community organizations to arrange for essential post-discharge services that guarantee continuity of care for your patients.
• Offering guidance and education for patients and their families about relevant treatment options and available resources.
• Facilitating crisis interventions and providing ongoing emotional support to patients and their loved ones as required.
• Participating in hospital committees, sharing your insights on social work practices, and contributing to the development of departmental policies.
• Maintaining accurate and up-to-date documentation in alignment with hospital standards, professional ethics, and legal requirements.

The focus of this role revolves around a diverse young patient population in school environments, addressing their unique psychosocial needs. You'll be instrumental in supporting students who may face varied challenges including mental health issues, family disruption, and developmental concerns. • A Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).

While the ideal candidate will have a minimum of two years of relevant experience in a healthcare or hospital setting, experience working with children or school-age populations is advantageous.
