Job description

US Army Reserve – Psychological Health Program Nurse Case Manager (RN)

The US Army Reserve Psychological Health Program (PHP) provides comprehensive behavioral health support to Reserve Soldiers and their families nationwide. As a Nurse Case Manager, you will manage clinical cases for Army Reserve Soldiers, coordinating treatment plans, advocating for Soldier readiness, and serving as the critical link between Soldiers, their providers, and their Commands. This primarily remote position is ideal for a certified nurse case manager passionate about behavioral health and military service.

Service Regions

This position supports Army Reserve units across four regional divisions nationwide:

  • 63rd Readiness Division – CA, NV, OR, WA, ID, MT, WY, UT, CO, AZ, NM, HI, AK, TX, OK
  • 81st Readiness Division – KY, TN, NC, SC, GA, FL, AL, MS, LA, AR
  • 88th Readiness Division – ND, SD, NE, KS, MN, IA, MO, WI, IL, IN, MI, OH
  • 99th Readiness Division – WV, VA, DC, MD, DE, NJ, PA, CT, RI, NY, MA, VT, NH, ME
Work Format

This is a primarily remote position (approximately 85% remote / 15% in-person). Services are delivered via a combination of telephonic counseling, virtual sessions, and periodic travel to Army Reserve installations within your assigned region. Federal (DoD) positions operate under federal practice authority.

Responsibilities

• Assessment: Receive requests for service and perform comprehensive assessments including financial resources, eligibility, social support, and profile initiation

• Planning: Assist Soldiers in establishing treatment plans that support optimal mental fitness and deployment readiness; provide condition and medication education

• Implementation: Facilitate access to care, advocate for Soldiers, and monitor treatment plan effectiveness

• Coordination: Coordinate with Soldiers, providers, and Commanders to ensure ongoing treatment engagement; report on readiness status

• Evaluation: Continually evaluate the need for continued services, Soldier progress, and readiness status

• Participate in Critical Incident response and ensure appropriate Traumatic Event Management services

• Support Yellow Ribbon events, Soldier Readiness Processing, and health promotion activities

• Investigate circumstances surrounding behavioral health issues and recommend prevention strategies when requested

• Maintain thorough case management documentation

• Travel to provide training and on-site services at Army Reserve installations

Qualifications

• Active, unrestricted Registered Nurse (RN) license

• Associate's Degree in Nursing (minimum) from an ACEN or CCNE-accredited program

• Required Certification – Must hold at least one of the following:

  • Certified Case Manager (CCM)
  • Certified Disability Management Specialist (CDMS)
  • Certified Rehabilitation Registered Nurse (CRRN)
  • Certified Occupational Health Nurse (COHN or COHN-S)
  • Advanced Certification in Continuity of Care (ACCC)
  • Certified Rehabilitation Counselor (CRC)
  • Nurse Case Manager (RN-NCM)
  • Care Manager Certified (CMC)

• Minimum 2 years of experience as a registered nurse case manager within the last 3 years

• Experience working in complex organizational environments

• Understanding of military operations, TRICARE, and VA health systems preferred

• Strong verbal and written communication skills

• Willingness to travel within assigned region (approximately 15%)
