Key Responsibilities
  • Conduct regular patient evaluations to assess and address their mental health needs effectively.
  • Facilitate communication among supervisors, other healthcare providers, family members, and guardians, ensuring collaborative care and progress reporting for each patient.
  • Develop a comprehensive treatment plan that includes appointments, home visits, and observation sessions, tracking all interactions to assess patient progress.
  • Organize and participate in home visits as necessary, which may involve conducting observations, reviewing charts, and interviewing staff to gather insights into patient care.
  • Ensure that program strategies and directives are implemented accurately and in a timely manner.
  • Take part in Interdisciplinary Team Meetings to foster collaborative treatment approaches and contribute to program development.
  • Prepare monthly and quarterly progress reports on patients, documenting treatment outcomes and areas for improvement.
  • Train team members on the execution of program plans and effective documentation practices, ensuring high standards of care.
  • Conduct regular follow-up interviews, home visits, and observations to ensure continuous, individualized patient support.
  • Manage patient records meticulously to ensure all information is organized and accessible for authorized personnel.
  • Act as a liaison between patients and the broader mental health community, helping connect individuals to essential resources for their recovery journey.
  • Contribute to the development of training materials aimed at improving program implementation and enhancing service delivery.
