This position is fully remote, allowing for a healthy work-life balance while still being a critical part of our Community Health Clinic team. • Act as a liaison between clients, families, and outpatient providers to facilitate comprehensive care coordination.

• Engage actively with insurance companies to initiate pre-certification and manage utilization effectively, ensuring that clients receive necessary services.

• Participate in discharge planning and aftercare services to promote long-term recovery, ensuring seamless transitions for clients exiting treatment.

• Deliver therapeutic meal support in group settings, modeling and promoting positive relationships with food and nutrition as a part of holistic care.

• Provide empathetic care tailored to the unique needs of clients, recognizing and affirming their intersecting identities and lived experiences.

Our client population encompasses individuals facing challenges related to addiction and co-occurring mental health disorders, with a particular focus on older adults. As an LCSW, you will employ culturally competent approaches to care, adapting interventions to align with the diverse backgrounds and identities of each client. Your role will be instrumental in fostering a safe and inclusive space for clients seeking recovery and personal development.

• A Master's degree in Social Work from an accredited institution is required.

• Candidate must possess a valid LCSW license in the state of Florida.

• Experience working with older adults or in a community health setting is preferred.

• Strong interpersonal and communication skills are necessary to build trusting relationships with clients and collaborate with multidisciplinary teams.

• Familiarity with telehealth platforms and electronic health record systems will be an advantage.

• Training in evidence-based therapeutic modalities, such as Cognitive Behavioral Therapy (CBT) or Motivational Interviewing (MI), is beneficial.
