• Act as a liaison between clients, families, and outpatient providers, ensuring seamless communication and coordination of care.
• Collaborate with insurance companies to manage pre-certifications and ensure appropriate utilization management for our clients.
• Engage in thorough discharge and aftercare planning, ensuring that clients transition smoothly out of our care and into community supports.
• Provide therapeutic meal support by modeling healthy relationships with food, reinforcing positive behaviors related to nutrition.
• Deliver safe, empathetic, and affirming care that respects the unique circumstances and intersecting identities of each client you serve.

This role focuses on individuals and families navigating the challenges of addiction and mental health disorders. You will work closely with clients of various ages, with a particular emphasis on older adults who may face unique barriers and needs in their recovery journeys. Your role will be critical in providing culturally competent care that respects and integrates clients' diverse backgrounds and experiences.

• A Master’s degree in Social Work from an accredited program is required.
• Possession of a valid LCSW license in the state of Florida is a must.
• Strong clinical and diagnostic skills are essential for effective practice in this dynamic field.
• Excellent communication and interpersonal skills, enabling you to build rapport with clients and collaborate with team members.
• Ability to work independently while also fostering strong team dynamics.

• Experience specifically working with older adults is highly desirable.

• Proficiency in electronic health record (EHR) systems for documentation and client management is an asset.
