Position Overview:

Supervises and oversees day-to-day activities, ensuring services address the psychosocial, emotional and medical needs of patients and/or a targeted population group to support optimal health and best clinical outcomes. Provides clinical supervision and intervention, and serves as a conduit between case management and social services to achieve safe, appropriate and timely discharges and/or patient movements to the next level of care along the continuum. Contributes to the achievement of key dashboard measures, ensuring appropriate care or referral to community resources is provided and resources are used cost effectively. Establishes work priorities and manages team workload and resources to meet operational demands and benchmark standards.

Job Description:

EDUCATION:

  • Master's: Social Work or related field


CERTIFICATION & LICENSURE:

  • LCSW-Licensed Clinical Social Worker


TYPICAL EXPERIENCE:

  • 5 years of recent relevant experience.


SKILLS AND KNOWLEDGE:

  • Working knowledge of health care delivery and case management processes in a managed care environment.
  • Knowledge of laws, regulations and professional standards affecting clinical social work and case management practices in an integrated delivery system (e.g., Centers for Medicare and Medicaid Services (CMS), Title 22, The Joint Commission (TJC)).
  • Working knowledge of available health care and community resources appropriate for populations served.
  • Knowledge of quality improvement methodologies.
  • Demonstrated leadership skills with the ability to mentor, motivate and engage team.
  • Demonstrates a proactive approach in identifying and addressing issues and concerns.
  • Ability to interpret and apply regulations, policies, procedures, practices, and recordkeeping requirements.
  • Ability to identify and analyze trends, determine solutions and resolve operational issues as guided by policies, procedures or practices.
  • Ability to establish trust and credibility, and to interact and maintain effective working relationships with those contacted in the performance of role's duties while respecting cultural and linguistic differences and fostering an inclusive work environment
  • Ability to communicate through verbal and written means, and to present information to a variety of audiences.
  • Ability to work effectively in a dynamic and fast-paced environment with changing business priorities.
