Job description

Center for Human Development, (CHD), is currently looking for Per Diem Behavioral Health Licensed Practical Nurses (LPN)ย to join the team at Heron House, an Enhanced Medical Group Living Environment (EMGLE) program. This 18-bed facility located in East Longmeadow, MA allows you to provide quality care and support to those we serve with a small LPN to client ratio. Under the supervision of designated charge nurse or RN, you will work with a support team of LPNโ€™s, CNAโ€™s and Direct Care staff providing care and compassion all the while gaining valuable experience in CHDโ€™s modern EMGLE facility.

A LITTLE ABOUT THE EMGLE FACILITY:

The Enhanced Medical GLE (EMGLE) provides a variety of skilled healthcare and supportive services including around the clock nursing and personal care assistance to Persons with mental illness in addition to compounding medical needs. These services are designed to meet and support the daily needs of Persons with chronic medical conditions, terminal illnesses and/or disabilities, which are impacted by their significant mental illness. These Persons require licensed and unlicensed professional services to meet their chronic complex medical and behavioral needs to ensure improvement or stabilization of their medical and behavioral condition. This model will also serve to maintain, prevent, or slow worsening of condition as well as assisting transition to a lower level of care setting or prevent hospitalization/nursing facility admissions.

A competitive hourly wage of $38.50/hr.ย With a $2.00/hr. differential for overnight shifts.ย 

Schedule: Per Diem - all shifts open for per diem.ย 

ย YOUR ROLE AS AN LPN YOU WILL:

  • Medication administration and management. Assist in medication audit processes
  • Attend participants care plan meetings/resource meetings
  • Provide direct care as needed. Providing personal hygiene care by assisting with travel to the bathroom; helping with showers and baths
  • Need to effectively understand details about a patientโ€™s health and provide charge nurse or RN with clear, precise information and observations
  • Provides activities of daily living by assisting with serving meals, feeding Persons as necessary, ambulating
  • Provides adjunct care by administering nonsterile dressings, surgical preps, ice packs, and heat treatments
  • Maintains the stability of Persons served by checking vital signs and weight, recording intake and output information
  • On call monthly rotation (stipend provided, plus hours wage if called in to pass meds)

Qualifications:

  • Have a license in practical nursing by The Board of Registration in Massachusetts
  • One-year experience as a LPN, preferred
  • One-year experience in a Behavioral Health setting, preferred
  • One-year experience working with the elderly, people with disabilities, behavioral challenges and cognitive disabilities, preferred
  • Valid driverโ€™s licenseย 
  • Meeting physical demands of the role. IE: Ability to lift 45 lbs. or more, stocking supplies, extended periods of walking and/or standing
  • Participate and successfully complete assigned training requirements in compliance with CHD and the Massachusetts Department of Mental Health
