Pay Range: $35 - $44 per hour based on experience.

Schedule: This is a PART TIME REMOTE position. Approximately 20 hours per week; 8am-5pm MOUNTAIN Time, days will vary as needed

What You Bring (Requirements)

  • Licensure: Current licensure in the state of Colorado as a Registered Nurse.
  • Experience: Minimum of two (2) years of direct MDS experience in a skilled nursing facility (SNF) with solid knowledge of PDMP, case mix systems, and reimbursement methodologies.
  • Certifications: Resident Assessment Coordinator – Certified (RAC-CT) certification required or must be obtained within six (6) months of hire. Resident Assessment Coordinator – Certified Advanced (RAC-CTA) required or must be obtained within twelve (12) months of hire.

What You’ll Be Responsible For

The MDS Coordinator is responsible for leading and coordinating the Resident Assessment Instrument (RAI) process to ensure the accurate, timely, and compliant completion of MDS assessments and care plans. This role supports regulatory compliance, reimbursement accuracy, and quality performance by working collaboratively with the interdisciplinary team, analyzing documentation and data, and driving continuous improvement in assessment and care planning practices. The MDS Coordinator is expected to fully utilize available systems and resources to support optimal clinical and financial outcomes aligned with organizational priorities.
