Job description

Stroke Data Abstractionist

Position Summary

Responsible for concurrent chart review and data abstraction for stroke patient records, reporting data, and providing feedback to Neuro Program Coordinators, maintain discharge call back program, and maintaining EMMI program.

Principal Accountabilities

Standards of Performance: Respect, Integrity, Compassion, Collaboration, Stewardship, Accountability, Quality

Education

Bachelor's degree in nursing (preferred) or associate's degree in nursing

Licenses and Certification

Current Illinois Registered Nurse

Experience and Skills

Technical Experience: 5 years RN experience in an acute setting.

Physical Activities

Intermittent hand manipulation required

Intermittent lifting and carrying of 20 lbs.

Role Specific Responsibilities

Maintains post discharge call back program including EMMI for all stroke patients

Primarily responsible for concurrent data abstraction of stroke patient records.

Provide feedback to nursing staff and Neuro Program Coordinators based off of data abstracted to help improve Stroke Program

Presents stroke data bimonthly at Stroke Committee Meetings

Report stroke data at CPIC meetings

Compensation (Commensurate with experience): $59,904.00 - $92,851.20
