Founded in 1970, East Valley Community Health Center is a Federally Qualified Health Center (FQHC) who's services include providing personalized, affordable, high-quality medical, dental, vision and behavioral health care through a community-based network within the East San Gabriel Valley and Pomona Communities. Our staff practices patient-centered care by serving each patient with a personalized care plan that meets their individual needs. Our patients have access to support services that include, nutrition, health education, case management, pharmacy, lab, and x-ray at our health center locations. East Valley serves the health care needs of uninsured and underserved individuals and families throughout our 8 health center locations.
Our mission is to provide access to excellent health care while engaging and empowering our patients, employees, and partners to improve their well-being and the health of our communities.
MAJOR POSITION RESPONSIBILITIES AND FUNCTIONS:
  • Facilitate and support quality improvement initiatives using PDSA cycles, root cause analysis, and process mapping.
  • Work directly with care teams and staff to execute improvement projects aligned with organizational, HRSA, PCMH, and payer priorities.
  • Assist teams in defining project aims, measures, timelines, and documentation.
  • Track, monitor, and report progress and outcomes of QI projects.
  • Support PCMH workflows including care coordination, access, team-based care, and population health initiatives.
  • Assist with compliance and improvement activities related to HRSA, UDS, and health plan quality programs (e.g., HEDIS, Medi-Cal managed care).
  • Support data collection, validation, basic analysis, and dashboard/report development.
  • Participate in Quality, Clinical, and Performance Improvement committees
  • Prepare documentation for audits, HRSA OSVs, and other regulatory reviews.
  • Provide coaching and technical assistance to staff on QI tools and improvement methods.
  • Other duties as assigned.

POSITION REQUIREMENTS AND QUALIFICATIONS:
  • Must have excellent interpersonal skills and empathy towards populations served by EVCHC.
  • Possess excellent communication skills, critical thinking skills, ability to handle stressful situations, capacity to function independently.
  • Must have excellent process improvement skills and be able to understand clinic functions and department interactions.
  • Knowledge of regulatory compliance.
  • Ability to work with high level positions and relate well to people from diverse ethnic and cultural backgrounds.
  • Ability to organize, analyze and synthesize complex data from various sources.
  • Able to understand and interpret financial statements.
  • Ability to evaluate process improvement projects and initiatives across the organization.
  • Willingness to work flexible hours to meet the organization's needs/demands.
  • Master's degree in business management with emphasis on Health Care Administration, or related master's degree is preferred.
  • FQHC experience is a must. A minimum of 5 years of progressively responsible administration experience in the health care industry, preferably serving underserved populations.
  • Thorough knowledge of the theory and practice of organizational management, preferably in a nonprofit environment.
  • Must be able to communicate effectively, write/speak succinctly.
