Hybrid teleworking option for eligible positions after probation Job Responsibilities Under general supervision, provides professional nursing care in accordance with established nursing protocols, policies, and procedures. Focuses on the prevention of illness, injury, and disability; the promotion of health; and the maintenance of the health of individuals, families, and populations. Performs clinical assessments, develops plans of care, provides treatment and health education, and coordinates services to improve health outcomes.

TYPICAL TASKS: Identifies high-risk populations and tailors interventions based on client needs and available resources. Performs comprehensive health assessments and evaluates healthcare needs of individuals, families, and populations. Develops, implements, and modifies plans of care based on assessment findings and client response to treatment.

Provides treatments, administers medications, immunizations, and other nursing interventions consistent with established protocols. Delivers preventive, maintenance, and health promotion services to diverse populations. Provides health education, counseling, and disease prevention instruction to individuals, families, and groups.

Screens clients and coordinates referrals to internal and external healthcare providers and community resources. Participates in the planning, implementation, and evaluation of public health programs and services. Applies principles of population health, disease prevention, and health promotion in clinical and community settings.

Develops and maintains collaborative relationships with community partners and stakeholders. Maintains accurate clinical documentation, reports, and records in accordance with agency standards. Participates in emergency preparedness, disaster response, mass dispensing, mass sheltering, and public health emergency activities.

Participates in training, quality improvement initiatives, and cross-training opportunities across program areas. Performs job responsibilities with minimal supervision while adhering to all agency policies and procedures. Performs other duties as assigned.

Minimum Qualifications Bachelor's degree in nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate's degree in nursing from an accredited college or university AND One year of nursing experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification. PREFERRED QUALIFICATIONS: Experience in public health nursing, community health, primary care, women's health, immunizations, communicable disease, HIV/AIDS services, family planning, or related clinical programs. Experience providing health education and case management services.

Bilingual skills (English/Spanish) preferred. Experience working with electronic health records and Microsoft Office applications. Knowledge of public health principles, disease prevention, and population health practices.
