LOCATION: Richardson Health Center DIVISION/DEPARTMENT: Clinical Services SALARY INFORMATION: $66,351.55 Annually / Pay Grade L / Job Code HCP191 Job Responsibilities Identifies high risk populations and tailor interventions based on the needs of the population and the resources available. Provides treatments, administers medications/vaccines and provides preventive and maintenance care to patient/families consistent with nurse protocols

Screens and refers clients to other community service providers. Participates in the planning, organizing and evaluation of program areas to ensure the goals and objective(s) of the program are met. Provides clinical services to target population.

Applies principles of population health to practice. Conducts comprehensive assessments of client's health care needs. Develops comprehensive plans of care for individual, families, and populations.

Modifies client treatment plans as indicated by client's responses and conditions. Provides health education instruction and/or classes to individuals, families, for the promotion and maintenance of health. Develops and sustains community partnerships.

Participates in drills and exercises on pandemic influenza, mass sheltering, and mass dispensing. Performs job responsibilities with minimal supervision. May manage specific program activities and/or specific program evaluations.

Adheres to all DCBOH policies and procedures. Minimum Qualifications Bachelor's degree in Nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate's degree in nursing from an accredited college or university AND One year of PHN experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR Certification. PREFERRED QUALIFICATIONS: Prior experience in a medical office setting with excellent customer service skills.

Public Health experience is a plus. Knowledge and usage of computers and Windows Software related programs. The ability to translate English to Spanish and Spanish to English orally and in writing is preferred but not required.
