This is a 6 month contract with possibility of extension or permanency. This individual will be responsible for leading a diverse, multidisciplinary team that will deliver high-quality, integrated care in the community, driving both operational and clinical excellence.

Job Description

The Program Manager, Integrated Care cultivates and manages high-performing teams comprised of Nurses, Personal Support Workers (PSWs), and Allied Health professionals – fostering a collaborative, innovative, and learning-focused environment that aligns with Spectrum's Mission, Vision, and Values.

In this role, you will ensure seamless program operations by leveraging data-driven insights to optimise service delivery, enhance quality, and achieve key outcomes. You will collaborate with external health system stakeholders to support the design and implementation of innovative programs, ensuring seamless care transitions across the broader health system.

  • Lead program development and design clinical workflows to support innovative models of care whilst managing key internal and external stakeholder relationships
  • Recruit, onboard, and mentor team members, ensuring they possess the necessary attributes, training, and skills to meet high care quality standards
  • Address quality, safety, and risk issues, monitoring key performance indicators and initiating improvements as required
  • Contribute to improved client outcomes through evidence-informed practices, resource efficiency, and timely resolution of client concerns
Qualifications
  • Current CNO certificate as Registered Nurse, degree preferred
  • Minimum of 5 years of recent experience with progressive responsibilities as a Nursing Manager, preferably in a community setting
  • Experience managing in a union environment is an asset
  • Solid knowledge of the principles, practices, and methods related to leading multidisciplinary teams
  • Experience with program development, implementation and evaluation
  • Valid G driver's license and reliable vehicle
