Job description

Service Coordinator

Provide case management and access to health services for senior residents.

Location: Park Ridge Commons

Schedule: Full Time Day

Hours: 8:30am-5pm

Provide case management and access to health services for senior residents. Key Responsibilities: Provides general case management (including intake) and referral services to all residents needing assistance. Provides formal case management (i.e. Evaluation of health, psychological and social needs, development of an individually tailored case plan for services and periodic reassessment of the resident's situation and needs) for a resident when such a service is not available through the general community. Establishes linkages with all agencies and service providers in the community; shops around to determine/develop the best cost in service pricing to insure individualized, flexible and creative services for the involved resident. Develops a directory of providers for use by both development staff and residents. Refers and links the residents of the development to services providers in the general community. Educates residents on service availability, application procedure, client rights and be an advocate as appropriate. Coordinate special wellness presentations relating to aging issues. Assists the residents build informal support network with other residents, family and friends. Responsible for staff training relating to aging and service coordination so that the team may better assist the residents. Monitors the ongoing provision of services from community agencies and communicates any changes in client status to the appropriate provider agency. Manages the provision of supportive services where appropriate. Assists Property Manager to ensure that safety rules and procedures are observed and implemented i.e. Lock-Out procedure, MSDS and all Local, State and Federal codes are observed and implemented during task assignments. Utilizes personal protection equipment, engineering and work practice controls. Track required data, complete and submit all required statistical reports i.e. the HUD grant performance reporting.

Minimum Qualifications: B.S. Social Work, Gerontology, Psychology or counseling is preferred, however individuals with appropriate work experience will be considered. Training related to aging, elder and disability services, and eligibility for state and federal entitlement program must be completed within the first year of service. Two or three years experience in social services for seniors and the disabled with familiarity of local resources.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
