The State of New Hampshire, Department of Military Affairs and Veterans Services has a full-time vacancy for Veteran Housing Program Coordinator.
Summary:
Plans, Coordinates and executes all aspects of the Ending Veteran Homelessness in New Hampshire (NH) Project for the State of New Hampshire. Leads integration of Veteran and military equities into federal, state, and municipal along with Veteran Service Organization and non-profit housing programs and initiatives that improve housing stability and wellness for Veterans, Service Members and their families.
YOUR EXPERIENCE COUNTS: Each additional year of approved formal education may be substituted for one year of required work experience and/or each additional year of approved work experience may be substituted for one year of required formal education.
MINIMUM QUALIFICATIONS:
Education/Experience: Bachelor's degree and 1 year of experience OR equivalent combination of 5 years of education and experience after completion of high school. Education and experience must be in social work, public administration, public health, mental health, housing stability, mitigating homelessness, substance use disorder, age friendly public health systems or related field.
License/Certification: Valid driver's license and/or access to transportation for statewide travel

Responsibilities:
DETAILED TASKS (AGENCY/POSITION-SPECIFIC):
In coordination with providers of services for veterans experiencing homelessness, is the State's program manager for the ending veteran homelessness through the "End Veteran homelessness in NH project charter".
Plans, coordinates, facilitates and oversees the conduct of the New Hampshire's "by weekly", "by name" Veteran Case Conferencing with federal and state agencies and Veteran Service Organizations and other care provider organizations Responsible for planning and conducting the annual case conferencing system mapping assessment and the annual care provider working group.
Plans, coordinates, and facilitates the execution of monthly ending Veteran homelessness in NH project's Veteran Steering committee composed of federal, state, and Veteran Service Organizations and leaders among the community of practice. Develops the context and identifies gaps in requirements and solutions to mitigate. Responsible for the planning and coordination of the annual ending Veteran homelessness in NH Summit for community of practice leaders from federal and state agencies and regional, state and local non-profits.
In coordination with local, state, and federal government agencies and nonprofits, facilitate development and in consultation with states and cities that have achieved an end to veteran homelessness develop and implement plans, programs, strategies and initiatives that include but are not limited to: coordinating a community outreach and a common assessment tool; develop a comprehensive listing of prioritized names for veterans experiencing homelessness; sharing data between all relevant veterans' service providers, state agencies, nonprofit organizations, and VA medical centers; increasing coordination and streamlining processes for appropriate veterans' housing placements and supportive "wrap around" services and connecting veterans experiencing homelessness to health, mental health, employment, and training.
Provide data driven reporting to DCBMP leadership on Veteran homelessness and in coordination with the community of practice facilitate identification of approaches and evidenced based solutions to end Veteran Homelessness to include sharing feedback with the NH Council on Housing Stability, the Department of Health and Human Services and NH State leadership. This feedback will inform respective stakeholder development of strategies, programs and initiatives and assist stakeholders and community partners with identifying, coordinating, and implementing regional community outreach plan and strategies to end Veteran homelessness.
Works with stakeholders including local, state and federal agencies, nonprofit organizations and 2-1-1 to increase awareness of resources available across the community of practice and the coordination and streamlining of processes for appropriate Veteran housing placements.
Identifies and monitors the top challenges facing Veteran homelessness such as affordable and available housing; mental health and substance use disorder; justice involved veterans and; aging veteran demographics in order to develop approaches and solutions to overcome these barriers.
Conducts financial actions associated with program management of housing program coordinator and initiatives. Conducts prospect research to identify, cultivate and solicit new funding and to build and promote sustainable partnerships for renewed support as well as seeks new partners whose funding priorities align with Service Member, Veteran and Family Members' Housing Stability needs.
