The Sycamore of River Falls is a growing, we are a full-service senior living community with 84 apartments including 68 blended independent and assisted living and 16 memory care apartments in a secured neighborhood on first floor. The property has four villa patio twin home units with two bedroom and three bedroom options for active, independent seniors. The apartment building consists of a variety of studios, one-bedroom, one-bedroom plus den, and two-bedroom units. The underground parking garage has 50 parking stalls, maintenance rooms, and residents’ storage area. Managed by Ebenezer. This newer care facility is Located in River Falls, WI

Ebenezer is hiring a Dimensions Program Manager. This role is accountable for implementing the Dimensions Program, encouraging participation and ownership from all staff. Manages and supervises the programmatic, physical and educational operations of Dimensions Community in meeting the resident's/family's needs and expectations. Collaborate with the interdisciplinary team and provides education to staff/families to adequately equip them to interact effectively with residents experiencing dementia.

Responsibilities

  • Provides leadership and supervision to staff, volunteers, and/or students.
  • Supports an effective staffing model and works with the Director of Health Services (DHS) and/or staffing designee to ensure efficient use of staffing resources.
  • In Partnership with the DHS, provide ongoing staff support.
  • In Partnership with the DHS, provide employee orientation and on-going education/staff development.
  • Provides resident/employee feedback to the DHS.
  • Advise on corrective action when appropriate.
  • Maintains department records for resident attendance at activities.
  • Implement Dimensions Program
  • Establishes and supports an environment conducive to the provision of quality services and high-level customer satisfaction.
  • Communicate and maintain good working relationships with residents, families, staff, etc.
  • Provide overall program direction and on-going development.
  • Develop monthly activity calendar for Dimensions neighborhood in collaboration with the Activities Director.
  • Responds effectively to customer concerns.
  • Routinely reviews and follows-up on satisfaction surveys.
  • Ensure proper programming supplies are available for residents, staff, etc.
  • Participates in Dimensions Program assessment.
  • Implement and maintain the Dimensions Program.
  • Serves as the Dimensions Champion for the site, attending Ebenezer’s Dimensions Trainings and ongoing Dimensions Champions training.
  • Coaches and mentors care team, other staff members and volunteers in best practices of the Dimensions Program to ensure effective approach and interaction with persons living with dementia. (e. not “living with memory loss”, as it’s not all about memory loss)
  • Collaborates with nursing to maximize recommended contributions to resident care plan from current Cognitive Performance Tests (CPT), My Way, Advance Care Planning and Global Deterioration Scale (GDS)
  • Works with the team to utilize psychosocial (i.e. non-pharmacological, such as aromatherapy, music, massage, etc.) approaches and interventions.
  • Ensures Dimensions environment is conducive to enhancing resident experience and encouraging engagement.
  • Facilitates, coordinates, and supports Site’s Care Partner Support group.
  • Sends monthly Dimensions Program newsletter to families.
  • Depending upon site, may manage the journey program
  • Develop program and activities reflective of Dimensions Community resident interests.
  • Assesses Dimension resident’s areas of interest and needs. Designs and implements programs to meet identified psychosocial needs of the residents.
  • Ensures, through observation and evaluation, that residents maintain their individualized highest level of performance and independence.
  • Plans, coordinates, and supports day-to-day activities and programming in Dimensions Community.
  • Plans and attends all Dimensions Program outings.
  • Ensure execution of programming through Activity Assistants, Resident Assistants and/or other available staff.
  • Encourages resident participation in activity programming.
  • Communicates and coordinates programming with Activity Director and other departments.
  • Assists the Activity Director in hiring and training Activities Assistants.
  • Offers suggestions and innovative ideas to improve the Dimensions Community to the Executive Director.
  • In Partnership with the Executive Director (ED) and DHS, develops budget, routinely monitors financial activities, and maintains accurate financial records.
  • Supports a marketing plan aimed at full occupancy.
  • Reviews Dimensions Community occupancy level regularly.
  • Ensures that marketing materials are current.
  • Coordinates marketing/PR initiatives as necessary to maintain full occupancy.
  • Participates in conducting resident/family tours of community.
  • Reviews clinical assessment for resident admission applications and may consult with team for resident placement.
  • Drives Ebenezer van/bus for resident outings:
  • Reviews and completes Vehicle Requirements – Transporting Residents Checklist prior to each resident outing.
  • Maintains a log of transportation services and tasks performed throughout the driving period.
  • Responsible for the safety and welfare of passengers.
  • Practices sound, safe judgment when performing transportation services including but not limited to: fire/disaster procedures and infection control procedures.
  • Recognizes and acts appropriately on safety hazards and incidents.
  • Follows all laws with regard to operating a motor vehicle.
  • Immediately reports any traffic violations or accidents.
