Position Overview
The Program Supervisor provides on-site supervision to his/her designated program and assists in overseeing the areas of individual support, medical needs, program support, and supervisory duties, while maintaining effective communication with appropriate team members and staff.


Essential Job Functions
1. Delivers quality and person-centered support services with dignity and respect for each individuals unique

needs, and advocates for individuals to be fully integrated into community life.

  • Demonstrates a person-centered attitude and builds positive relationships with the individuals supported by providing choices and promoting independence.
  • Assists each individual in meeting personal desires by encouraging participation in leisure and recreational activities in the community and ensuring adequate staff coverage for scheduled activities.
  • Supports people in establishing and maintaining relationships with friends, family, and community connections.
  • Promotes each persons dignity of risk while following through with Individual Abuse Prevention Plans (IAPPs), service outcomes, and positive behavioral support plans to reduce vulnerability in identified areas.
  • Monitors the daily activities of individuals supported and collaborates with the Program Coordinator to address any concerns in a timely fashion.
  • Collaborates with other providers and assesses the need for additional support recommendations.
  • Gathers necessary information and documentation to conduct and participate in team meetings and annual reviews.
  • Assures the Coordinated Service and Support Plans (CSSPs) and Coordinated Service and Support Plan (CSSP) Addendums are implemented properly.
  • Ensures central files and individuals books are up to date.


2. Collaborates with Program Nurse and Program Coordinator to manage the medical needs of supported individuals, and ensures staff are appropriately trained regarding the core needs of the individuals in the program/s.

  • Follows the Health Plans as written for each person and documents appropriately.
  • Administers and documents medications and treatments as directed.
  • Follows procedures utilizing medical equipment; reports malfunctions or needed repairs.
  • Locates and uses medical information and resources established by the program.
  • Monitors and documents individuals health concerns and changes, and consults with Program Nurse, and/or Director of Nursing and Program Coordinator.
  • Consults with nursing staff regarding medical recommendations, such as Physical Therapy, Occupational Therapy, Speech Therapy, etc.
  • Assists Program Nurse in scheduling and attending dental and certain medical appointments including but not limited to Physical Therapy, Occupational Therapy, Blood Draws, Talk Therapy and any other appointments deemed appropriate by Director of Nursing and/or Program Director.


3. Oversees program operations.

  • Assists with coordination of Individual intake and/or discharge processes.
  • Assists Program Coordinator in preparing for county licensing reviews.
  • Collaborates with Program Coordinator to ensure Program Abuse Prevention Plans (PAPPs) are up to date and implemented properly.
  • Completes necessary documentation General Event Report Form (GER) Behavioral Intervention Report Forms (BIRFs).
  • Ensures adequate staff coverage and assists with filling open hours to maintain program ratios and communicates discrepancies to Program Coordinator.
  • Maintains and posts weekend staffing schedule and assists Program Coordinator in reviewing/responding to time off requests.
  • Assist Program Coordinator with Monthly/Bi-Monthly staff meetings. Helps facilitate information to absent employees.
  • Facilities coverage of program when Program Coordinator is off. Working with Program Manager/Director as needed.
  • Assists with monitoring program budgets and personal needs accounts for appropriate use, and ensures expenses stay within budgeted allotment.
  • Ensures vouchers are completed accurately and in a timely fashion, and receipts are attached for all individual and program expenses before being turned into Program Coordinator.
  • Oversees completion of menu preparation and grocery/supply shopping.
  • Reports program maintenance needs to the Property Director and Program Coordinator.
  • Follows Hiawatha Homes transportation guidelines when operating vehicles.
  • Ensures general vehicle maintenance is performed routinely and brings any concerns to the Property Manager.
  • Facilitates quarterly weather and fire drills and keeps supplies in the emergency preparedness kit up to date.
  • Prepares monthly Activity Calendar for HH
  • Prepares and posts activities for week specific to FSS program, on activity board.
  • Post updates/communication items through email, Therap etc.
  • Tracks billing log by maintaining it on biweekly basis and submitting by the 5th of every month.
  • Bring any schedule needs or changes with individuals to Program Coordinator.


4. Oversees supervision of staff.

  • Creates and maintains a positive and safe work environment and culture and acts as a positive role model to program staff.
  • Participates in orientating, and training Direct Support Professionals/Aides.
  • Provides direct supervision of and ongoing feedback to Direct Support Professionals/Aides and communicates any performance concerns with Program Coordinator.
  • Ensures compliance with agency policies and practices.
  • Implements cleaning guidelines and provides a sanitary and safe environment for each individual. This includes, but is not limited to cleaning wheelchairs, laundry, bedrooms, bathrooms, and other locations.
  • Participates in designated weekend on-call duties.
  • Will be required to work a rotating weekend shift.
  • Collaborate with Program Coordinator to address employee performance concerns promptly.
  • Adds day-to-day feedback to Evaluation Work Period Reviews and Performance Evaluations.
  • Participates in Leadership Meetings and relays pertinent information to Direct Support Professionals/Aides.


5. Maintains appropriate and effective communication.

  • Assists Program Coordinator in monitoring Therap and E-MAR.
  • Checks agency email account, voicemail, and mail folders regularly, and responds appropriately.
  • Maintains calendar on Outlook to accurately reflect work schedule.
  • Maintains communication with parents/guardians, Case Managers and other applicable team members when directed by Program Coordinator.
  • Communicate with Program Coordinator regarding the general functioning of the program.


6. Other

  • Will have a regular 80 hours work agreement with a schedule made to best support the program including every other weekend.
  • Will complete 64 hours of DSP per pay period and 16 hours of Admin per pay period.
  • Arrives on time, works scheduled shifts, and maintains shift coverage until relief staff arrives.
  • Assists with temporary coverage of other programs as needed or as designated by Program Manager.
  • Complies with Hiawatha Homes' staff development requirements and attends/participates in professional development as needed or directed.
  • Ensures annual training compliance is completed, and all other required trainings are current (CPR, Behavioral Supports Training, Back Safety, etc.).
  • Completes other duties as assigned.


Minimum Job Requirements

  • Is at least 18 years of age.
  • Has the ability to speak, write, and comprehend the English language.
  • Possesses a valid drivers license, satisfactory motor vehicle record, and successfully completes a Hiawatha Homes driving test.
  • Is able to provide proof of employment eligibility and proof of being free from tuberculosis upon hire.
  • Successfully passes a State of Minnesota background check before working unsupervised.
  • Is willing and able to work with and support both males and females.
  • Successfully completes the Hiawatha Homes Medication Administration class.
  • Possess a High School Diploma/GED and 2 years of DSP experience or 1 year DSP experience and 1 year supervisory experience in a related field.
