The Unite Us Program Coordinator is responsible for the day-to-day management and growth of the Unite Us coordinated care network for Social Determinants of Health (SDOH) across Palm Beach County. This role serves as the primary point of contact between the Health Care District, community-based organizations, and the Unite Us vendor team, ensuring the platform is fully utilized, properly maintained, and continuously expanded to meet the evolving needs of residents and partner agencies.
The Coordinator drives partner enrollment, monitors platform adoption and data quality, manages the active resource directory, and works closely with the vendor to troubleshoot issues, roll out new features, and onboard new services. Success in this role is measured by network growth, consistent and accurate platform usage among partners, and the overall health of the closed-loop referral system.
In addition, the position provides day-to-day coordination and administrative support for the District's Sponsored Programs initiative, including funding application administration, contract and reporting support, partner communication, payment coordination, data tracking, committee support, and maintenance of reporting systems. The position assists with program monitoring, community partner engagement, and implementation of initiatives designed to improve community health outcomes and access to services.
Key Responsibilities
Partner Enrollment & Network Growth
Platform Utilization & Adoption
- Monitor partner activity and usage metrics to ensure organizations are actively sending, receiving, and closing the loop on referrals.
- Identify low-engagement or inactive partners and develop targeted outreach and re-engagement strategies.
- Provide ongoing technical assistance, refresher training, and best-practice guidance to current users.
- Track and report on key performance indicators, including referral volume, response times, and outcome completion rates.
Resource Directory Maintenance
- Maintain accurate, up-to-date program and service listings within the platform's resource directory.
- Conduct regular audits of active resources to verify eligibility criteria, capacity, contact information, and service availability.
- Coordinate with partner agencies to update or retire outdated listings and add new services as they become available.
Vendor Relationship Management
- Serve as the primary liaison with the Unite Us vendor team for platform support, troubleshooting, and account management.
- Coordinate implementation of new platform features, modules, and service lines (e.g., new care coordination pathways or program-specific networks).
- Participate in regular check-ins with the vendor to review performance data, escalate issues, and plan upcoming enhancements.
- Manage contract deliverables, renewal timelines, and licensing/seat allocations in coordination with finance and procurement.
Program Administration & Reporting
- Prepare regular reports and dashboards on network performance for leadership, the Board, and funding partners.
- Support grant reporting and compliance requirements related to SDOH and care coordination initiatives.
- Develop and maintain documentation, including standard operating procedures, training materials, and partner agreements.
- Stay current on best practices, emerging needs, and policy developments related to SDOH and coordinated care networks.
Sponsored Programs
- Assist with administration of the District's Sponsored Programs initiative, including application intake, document review, reporting follow-up, and maintenance of program records.
- Assist with contract routing, payment coordination, invoice tracking, and monitoring reporting submissions.
- Assist with website updates, application portal support, and coordination of electronic application activities.
- Prepare meeting materials, agendas, minutes, reports, presentations, and committee support documentation.
- Respond to inquiries in a professional, timely, and customer-service-oriented manner.
- Participate in process improvement and operational enhancement activities related to community coordination, care navigation, and Sponsored Programs initiatives.
- Ensure adherence to confidentiality requirements, organizational policies, and applicable HIPAA/privacy standards.
- Emergency duty may be required of the incumbent, including responses to disasters, emergencies, shelters, or other District emergency operations as assigned.

Education:
- Bachelor's degree in public health, social work, healthcare administration, or a related field, or equivalent combination of education and experience.
Experience:ย
- Minimum two (2) to three (3) years of experience in community health, care coordination, social services, or program/account management.
- Demonstrated experience managing partner or vendor relationships and driving adoption of a software platform or program.
- Strong understanding of Social Determinants of Health and the community-based organization landscape in Palm Beach County (preferred).
- Excellent verbal and written communication skills, with the ability to train and support users with varying levels of technical comfort.
- Strong organizational skills with the ability to manage multiple partner relationships and projects simultaneously.
- Proficiency with Microsoft Office and comfort learning new software platforms; direct experience with Unite Us is a plus.
- Valid Florida driver's license and ability to travel locally to partner sites and community events.
Preferred Qualifications
- Direct experience administering or coordinating a Unite Us network or similar coordinated care/referral platform.
- Established relationships with social service, healthcare, or nonprofit organizations in Palm Beach County.
- Experience with data analysis or reporting tools to track program performance and outcomes.
- Bilingual (English/Spanish or English/Creole) strongly preferred.
