Job description

Day-to-day responsibilities of this position:

  • Organizing and scheduling annual Individual Program Planning (IPP) for up to 70 individuals.
  • Writing annual evaluations for the IPP.
  • Teaching, training, and gathering new information on rights for people with disabilities.
  • Participating as a team member on the Treatment Team.
  • Coordinating legal and guardianship issues for assigned caseload.  
  • Individual Rights advocate. Train individuals and staff, monitors for rights violations, resolves individuals grievances, attends Human Right meetings and coordinates other meetings.  
  • Protected Health Information (PHI). Trains employees and staff on PHI, gets guardians to complete PHI consent forms.
  • Admissions, transfers and discharges. Attends admission meetings as assigned, completes necessary evaluations, coordinates transfers and discharges, writes discharge summaries.  
  • Assist with funeral and end of life planning. Coordinating with the guardian and family at the time of death. Provide education and resources for staff.
