Job description

  •  Coordinate the daily operation and delivery of Day Habilitation services in accordance with NJ DDD regulations and agency policies.
  •  Supervise and provide guidance to Direct Support Professionals, Client Coordinators, and designated supervisory staff.
  •  Ensure adequate staffing levels and staff-to-participant ratios in compliance with regulatory requirements.
  • Recruit, train, orient, coach, and evaluate staff; identify ongoing training needs and maintain required staff certifications and training records.
  • Ensure person-centered Day Habilitation Plans are implemented, monitored, reviewed, and updated as needed to support participant outcomes.
  •  Monitor participant progress, review documentation, and ensure timely completion of progress notes, assessments, and required program records.
  •  Participate in Individualized Service Plan (ISP) meetings and collaborate with participants, families, support coordinators, and interdisciplinary team members to coordinate services.
  •  Review and ensure the accuracy and completeness of participant documentation and regulatory records.
  • Ensure incidents are reported, documented, investigated, and followed up in accordance with agency policy and NJ DDD requirements.
  • Oversee emergency preparedness activities, including fire and evacuation drills, and maintain all required documentation.
  •  Conduct regular staff meetings and communicate program expectations, policy updates, and quality improvement initiatives.
  •  Monitor program operations to ensure participant health, safety, rights, community inclusion, and quality of services.
  •  Assist with program budgeting, resource management, and administrative functions as assigned.
  • Collaborate with agency leadership to support program development, regulatory compliance,and continuous quality improvement.
  • Perform other related duties as assigned

  • Bachelors degree in the Human Services related field, with one (1) year of experience in related work experience. Associate's degree, with (5) years of related work experience or a High School Diploma/GED, with 10 years of work experience with the developmentally disabled community.
