Job description

A Home Health Intake Coordinator manages the initial patient referral process, acting as the key liaison between hospitals, physicians, families, and clinical staff to ensure a seamless start of care. They are responsible for receiving referrals, verifying insurance benefits, obtaining authorizations, and accurately entering patient data into Electronic Medical Records (EMR).


As a Intake Referral Coordinator, you will:

  • Receives and appropriately documents new referrals in a timely manner.

  • Verifies each patient’s payor source and maintains and tracks insurance authorizations and initial certifications.

  • Participates in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group.

  • Provides effective communication to patients, staff members, other health care professionals and referral sources.

  • Responds to Agency and patient needs in a professional and creative manner.

  • Demonstrates commitment, professional growth and competency.

  • Participates in Agency-sponsored in-service training.

  • Participates in the Agency’s QAPI program.

  • Promotes the Agency philosophy and administrative policies to ensure quality of care.


Requirements

  • Minimum one year of experience in referral intake is preferred

  • Experience with third party payers, Medicare and Medicaid highly desirable

  • Medical Field background preferred, including familiarity with medical terminology

  • Excellent computer and customer service skills

  • LVN/LPN preferred

  • Valid Driver’s License

  • Valid Auto Insurance

  • HomeCare HomeBase experience preferred
